Cargo Handling Equipment Survey for Ports and Intermodal Rail Yards

In mid-December 2004, we distributed a Mobile Cargo Handling Equipment Survey to ports and intermodal rail yards statewide. We are also making the survey available on this website for those who wish to complete the form electronically (to submit it via e-mail, or to print and mail/fax it to us).

We are providing the survey in two formats. If you do not have Microsoft Word, you may use the Adobe Acrobat version. If you wish to complete the survey electronically and save it to your hard drive or disk (i.e., if submitting the survey via e-mail), you must use the Microsoft Word format.
