Please note that the Preliminary Form due date is quickly approaching; it must be completed and returned to the ARB by November 24, 2004. The survey forms must be completed by all responsible parties and formulators, and are due to the ARB by March 15, 2005. We have developed and are making available two electronic versions of the survey, one specifically designed for responsible parties and one specifically designed for formulators. The survey packet (page III-8) contains instructions on how to request and complete the electronic version of the survey.
