Designation of Persons Authorized to Receive Warrants

One of the most important items in every employee's Official Personnel File (OPF) is the Designation of Persons Authorized to Receive Warrants" form (Std. 243). Why is this completed form so imporant? The information on this form provides the Air Resources Board (ARB) with the name and social security number of the person you wish to receive your final pay and any vacation/CTO/holiday pay you have on the books in the event of your death while employed by ARB.

Std. 243 is a form that is given to each new employee to complete when they are first appointed into State service. It is the employee's responsibility to complete a new form whenever they have a major life change (e.g., marriage, divorce, birth or adoption of a child, etc.).

It is important to note that if you do not have a current Std. 243 on file in your OPF in the event of your death, your pay will be released to survivors in the following order:

  1. Surviving spouse
  2. Natural and adopted children
  3. Parents
  4. Brothers and sisters
  5. Your estate
  6. Your trust
  7. Stepchildren
  8. Grandchildren
  9. Nieces and nephews
  10. Great-grandchildren
  11. Cousin

Some of our employees do not have a Std. 243 on file in their OPF. In order to insure that every employee ha this crucial form on file and that the information on the form is current, the Office of Human Resources is asking all employees to complete a new Std. 243 in duplicate as required by law, and submit it to your assigned Personnel Specialist (PSS) immediately.

You can download, complete, and print Std. 243 and submit it to your PSS. Remember to submit the form in duplicate and to sign each copy.

Persons Authorized to Receive Benefits Upon Your Death

If you die before you retire, CalPERS law provides for payment of specific benefits to your surviving beneficiaries. For CalPERS benefit purposes, if you wish to designate beneficiaries other than the statutory beneficiaries identified above or in a different order, you must complete Std. 241.
